Celebration of Life Service for Ms. Phyllis D. Thomas will be held on Saturday, April 8, 2017, at 3 p.m. at Smith Grove Missionary Baptist Church located at 7136 Tallahassee Highway, Attapulgus, Ga. with Rev. Dr. Charles Grice, officiating. Ms. Thomas will lie in state on Thursday, April 6, 2017, from 1 p.m. – 7 p.m. at Q.L. Douglas Funeral Home. Southside Chapel 4105 St. Augustine Road Jacksonville , Florida. Visitation will also be held on Friday, April 7, 2017, from 9 a.m. – 7 p.m. at Bouie Memorial Chapel of the Collins Family Mortuary. Internment will follow in Sherman Williams Cemetery.
